LDCT Worksheet & Best Practice Reminders
In January, we updated our LDCT worksheet to capture more of the data requested by the ACR within the Lung Cancer Screening Registry, and to note the reminder that when performing a Low-Dose CT of the Chest for a lung cancer screening, the patient should be asymptomatic.
We ask that you use this worksheet with every LDCT to ensure appropriate screenings and to capture complete data for submission to the registry. We no longer need the ACR's Case Registration Form or Exam Form as this worksheet captures that data.

Some best practice reminders for Lung Cancer Screening LDCTs:
- Medicare patient eligibility requirements:
- Age 55 - 77 years
- Asymptomatic (no signs or symptoms of lung cancer)
- Tobacco smoking history of at least 30 pack-years
One pack-year = smoking one pack (20 cigarettes) per day for one year
- Current smoker or one who has quit smoking within the last 15 years
- When the patient or a family member completes the LDCT worksheet themselves (which is totally fine), we're finding that some of the required documentation is either left out altogether or documented incorrectly. We ask that you please review each LDCT worksheet for required documentation of:
- Did physician provide smoking cessation guidance?
- Is there documentation of shared decision making?
- Number of pack-years of smoking smoking (cigarettes)
